New Patients

Welcome to Steward Family Medicine. Dr. Steward, a board certified Family Medicine physician, and Chris, a Certified Medical Assistant and Phlebotomist, make up our staff. Our practice goal is simple: Provide exactly the care you need exactly when you need it. If you are interested in becoming a patient in our practice, we ask that you please review the Office Policies link where you will find information on e-mail, HIPPA laws, and our practice policies. For your initial visit with us, please e-mail (or call) the office to make an appointment.

Before your first visit, please:

  1. Read the items under Office Policies.
  2. Go to the Signature Page and print it out. Please remember to bring it with you.
  3. Click to fill out the: Secure New Patient Information Questionnaire Survey. Having this information before your visit allows Dr. Steward to use your appointment time more effectively.
